Social Worker - Disability Waiver Case Manager

Otter Tail County is accepting applications for a Social Worker - Disability Waiver Case Manager.  

The purpose of this position is to provide case management services and assessment of need to children and adults with complex needs and disability waiver case management.  Social Workers establish collaborative relationships with people served, their families and/or legal representatives, service providers, and other agency staff.  They participate and assist with the implementation of team, inter-departmental team and agency goals as well as policies and procedures to provide optimal services to people based on resources available to the agency.
