Public Health - Long Term Care Nurse

Otter Tail County is accepting applications for a full-time Long-Term Care Nurse.

Otter Tail County Public Health provides population-based public health nursing services that promote, protect, and improve the health of individuals, families, and communities. This position assesses community health needs, emphasizes prevention and social determinants of health, collaborates with community partners, and supports public health initiatives.  This includes advocacy, policy development, and health planning activities in alignment with public health standards and community health priorities.

This position is part of the Long-Term Care Unit and is assigned to the Interdisciplinary Public Health/Human Services Unit and includes, but is not limited to, providing care coordination to persons who are enrolled in managed care health plans.  Care coordination includes assessment, care plan development, arranging and/or coordinating supports and services which may include preventative, routine, and/or long-term care health and service needs.
